Pre-Sorted Packets
Medications are sorted into individual packets labeled by time of day. morning, noon, evening, and bedtime. so you always know exactly what to take and when.
Detailed Labeling
Each packet includes the name, strength, and dosage of every drug, plus a physical description (color, shape, and imprint) so you can verify what you are taking.
Medication Teaching
Your nurse explains what each medication does, why you are taking it, and what side effects to watch for. turning a confusing list into an understandable plan.
Medication Reconciliation
We review all your medications. including over-the-counter drugs and supplements. to identify duplicates, interactions, and unnecessary prescriptions.
Pharmacy Coordination
We coordinate with your pharmacy and physician to ensure all prescriptions are current, refills are on time, and any changes are communicated clearly.
