What Is the GUIDE Model?

GUIDE stands for Guiding an Improved Dementia Experience. It is a voluntary, nationwide initiative by the Centers for Medicare & Medicaid Services (CMS) designed to improve the care and support for individuals with dementia and their family caregivers.

The program began on July 1, 2024 and is scheduled to run for 8 years. It addresses the challenges of fragmented dementia care and the significant physical, emotional, and financial burdens faced by family caregivers. providing coordinated support that keeps people with dementia safely at home longer.

What GUIDE Provides

We are a GUIDE participant. Here is everything included in the program at no cost to you.

Dementia Care Navigation
A dedicated dementia care navigation professional guides your family through every stage. from diagnosis to daily care decisions.
24/7 Support Line
Around-the-clock access to a support line for patients and caregivers. immediate help when a crisis occurs, day or night.
Respite Care. Up to $2,500/Year
Up to 83 hours of in-home personal care, 9 days of assisted living respite, or 32 days of adult daycare. paid by Medicare to give family caregivers a break.
Caregiver Education
Expertly developed, evidence-based education for family caregivers. covering communication techniques, safety, behavioral management, and self-care.
Community Resource Connections
Your care navigator connects you to local doctors, nurses, social workers, and community health resources. eliminating the burden of finding help on your own.
Home Safety Assessment
A thorough assessment of fall risks, wandering hazards, and other safety concerns in the home. with specific recommendations for modifications.
Medication Education
Guidance on dementia medications. what they do, side effects to watch for, and how to manage complex regimens safely.
Delays Nursing Home Placement
Coordinated dementia care enables people to remain safely at home longer. preventing or delaying costly nursing home admissions.

Paid Respite for Family Caregivers

Caring for a loved one with dementia is one of the most demanding roles a person can take on. GUIDE provides paid respite so caregivers can rest, recharge, and continue providing care.

Who Qualifies for GUIDE?

1.You are a family caregiver for a loved one with a dementia diagnosis
2.Your loved one is a Medicare beneficiary
3.Your loved one is living at home (not in a nursing facility)
4.Your loved one has not been formally enrolled in hospice
