Eligibility

Do You Qualify?

To receive Medicare home health care, you must meet all four criteria set by the Centers for Medicare & Medicaid Services (CMS):

You are considered homebound — it is difficult or tiring for you to leave home without assistance.

You require intermittent skilled nursing care, physical therapy, speech-language pathology, or continued occupational therapy.
3
You are under the care of a physician, nurse practitioner, or clinical nurse specialist who certifies your need for home health care.
4
You receive care from a Medicare-certified home health agency.
